How to obtain an NPI (National Provider Identifier)

(National Provider Identifier.) If you are a provider of medical services covered by HIPAA (Health Insurance Portability and Accountability Act), you are required to obtain an NPI. Visit the NPPES (National Plan and System Provider Enumeration) website: https://nppes.cms.hhs.gov/NPPES/StaticForward.do?forward=static.instructions
2 Click the link for “National Provider Identifier.”
3 Select the option “Apply online for an NPI.”
4 Click the option at the bottom of the page for “Begin Application Form.”
5 Enter the required information. You will need your Social Security number, bequest number, birth certificate information, contact information, license number, EIN, and taxonomy code. Submit your application after all the information currently available.
6 Look for their number in the mail. You should receive the NPI number within 15 days of submitting your application online.
7 Contact the NPI enumerator at 800-465-3203 to have the application by mail.
